2025 Community Health Survey Begins

Adult residents of Jefferson, Lewis, and St. Lawrence counties encouraged to share their thoughts regarding local healthcare


Fort Drum Regional Health Planning Organization (FDRHPO), in partnership with the North Country Health Compass Partners, will begin outreach to community members for the 2025 Community Health Survey on Monday, June 2nd.

 

This year marks the 10th annual survey in which residents of Jefferson, Lewis, and St. Lawrence counties are asked to share their experiences with local healthcare, overall health, and lifestyle habits. For 2025, new questions regarding social determinants of health (SDOH) have been included to align with the updated New York State Department of Health (NYSDOH) Prevention Agenda for 2025-2030.

 

Conducted annually, the survey gathers adults' thoughts and experiences regarding healthcare. Responses remain anonymous and are utilized to evaluate the effectiveness of local programs, identify specific health needs within the community, and develop plans to address these needs.

 

The survey methodology uses randomized email invitations and MMS push-to-web sampling to reach residents throughout the tri-county region. FDRHPO aims to collect at least 1,200 anonymous responses from adults living in the North Country.
